Collection of Information
We collect and use information about you in several ways. We receive information directly from you when you send us an email, share a photo, complete a form on our websites (such as a survey or a request for a brochure or a contest entry), sign up to receive email from us, or talk to us via text, chat or phone. We collect information electronically when you browse our site.

You are not required to accept any cookies to use our websites. We use third-party cookies (such as the Google Analytics cookies) to gain insights about the demographics and usage behavior of our visitors and to serve advertising to them across the Internet.
Personal information is information that can be used to identify you individually, such as your name and phone number. We collect personal information only when you explicitly give it to us in form fields on the page. Completing the form fields gives My Healthy Baby and MOMs Helpline permission to call you. The contact information you provide will be used to call you regarding a local program referral. The Indiana Department of Health will also use the referral information to evaluate our referral process. Your personal information will not be used in any reports on this process. If you wish to revoke permission for My Healthy Baby and MOMs Helpline to call you, please call 844-624-6667.
Data Processors We Use, Use of Collected Data, and Opting Out
We utilize products that collect, receive or use end-users’ personal data. Data is collected to allow us to provide our services, as well as for remarketing and behavioral targeting, advertising, analytics, and email marketing. Personal data is collected using the following services:

We utilize advertising products offered by Facebook and its affiliates, potentially including Instagram, Oculus and WhatsApp, that collect, receive, or use end users’ personal data. Facebook Ads conversion tracking is an analytics service provided by Facebook, Inc. that connects data from the Facebook advertising network with actions performed on our website. You can generally opt-out of receiving personalized ads from third party advertisers who follow the Digital Advertising Alliance’s Self-Regulatory Principles for Online Behavioral Advertising by visiting the opt-out page on the DAA site.

Information We Share
We may disclose statistical information to third parties in aggregate form without revealing any personal details about our users (for example, 60% of our visitors live in Marion County).
We do not share personal information you voluntarily provide, except as necessary to provide support and services. We may disclose your personal information as required by law, in conjunction with a government inquiry, or in litigation or dispute resolution. We may disclose your personal information to our trusted service providers who work on our behalf, do not use the information we disclose to them except as necessary to provide support and services to you, and have agreed to adhere to the rules set forth in this privacy statement. Lastly, we may disclose your personal information when we believe in good faith that disclosure is necessary to protect your safety or the safety of others.
Access to Your Data

We have appropriate security measures in place in our physical facilities to protect against the loss, misuse or alteration of information that we have collected from you on our site. Please note that although we take reasonable steps to protect your information, no website or computer system or wireless connection is completely secure.

If our information practices change at some time in the future, we will post the policy changes here and provide you with the ability to opt out of these new uses.
